Overview

Hon'ble Prime Minister, Shri Narendra Modi has launched the COVID-19 vaccination drive on 16 January 2021. This marks the beginning of the world’s largest Coronavirus vaccination program in India.

  • What are some relief measures for the startup affected by COVID?

    1. The Finance Minister announced a number of relief measures to ease the regulatory and compliance burden of businesses including extension of timelines for filing income tax returns for (FY 18-19) from 31st March, 2020  to  30th June, 2020 and other due dates for issue  of notice, intimation, notification, approval order, sanction order, filing of appeal, furnishing of return etc. under Income Tax Act,  Wealth Tax Act etc. where the time limit is expiring between 20th March 2020  to 29th June 2020 shall be extended to 30th June 2020. Further relaxation is provided on interest rate for delayed payments of advanced tax, self-assessment tax,  regular tax, TDS etc.  made between 20th March 2020  and  30th June 2020,  reduced interest rate  at 9%  instead of 12 %/18 % per annum ( i.e. 0.75% per month instead of 1/1.5 percent per month). No late fee/penalty shall be charged for delay relating to this period.

    2. Certain relaxations have also been provided under the Companies Act to reduce the compliance burden, including financial burden of companies/ LLPs at large sch as waiver of additional fee for late filing etc.

    3. An economic package of Rs. 1.7 lakh crore to address the present economic breakdown occurred due to spread of the highly contagious coronavirus or Covid-19 infection was announced on 26th March, 2020 covering Insurance scheme for Health Workers, Food Security and Direct Benefit Transfer.

    • The primary challenge in such times is to manage available cash and create avenues to generate cash.
    • To manage available cash, streamline your expenses.     • Cash is generated from existing resources and from existing small assets as well (FDs, NSCs, gold, etc.) which businesses and individuals create active economic times when there are surplus incomes. The point is once the initial period is handled and situations stabilise, your regular cash flows shall enable you to once again create such assets.     • You can request some of your service providers for staggered payments or for a postponement of payments.
    • The last option could be approaching a lending institution (your existing lender or a small finance bank which specialise in lending to small businesses) for emergency cash flow – but please bear in mind that while a new loan will bring in cash, it will also bring the interest burden which demands cash outgo. Be also very clear about the loan servicing required and your ability to take care of it. Ideally, this is best resorted to – if at all - when situation becomes clearer and activities restart and are stable.     You can get non-refundable withdrawal to the extent of the basic wages and dearness allowances for three months or up to 75% of the amount standing to your credit in the EPF account, whichever is less. Credit under GECL would be up to 20% of the borrower’s total outstanding credit up to Rs.25 crore, excluding off-balance sheet and non-fund based exposures , as on 29th February, 2020, i.e., additional credit shall be up to Rs. 5 crore.

    In our experience, there have been no delayed post-COVID thrombotic complications. Since the prothrombotic state parallels the viremic and inflammatory phase, once the patient has been discharged, the thrombotic risk also decreases. So, we don’t recommend anti-coagulation at discharge in routine COVID patients unless indicated for other reasons like DVT, prosthetic valve etc.

  • Sudden deaths in COVID-19

    Sudden deaths have been reported both at presentation to emergency department (ED) as well as in hospital. Reasons that have been proposed include a sudden cardiac event/ ACS, preceding silent hypoxia that went unnoticed or due to a thrombotic complication such as pulmonary thromboembolism. Patients with risk factors to develop severe COVID or with prior comorbid conditions such as CAD or chronic lung disease should be strictly monitored for their saturation. They should not be allowed to move unattended. Anticoagulant should be used in all at-risk patients who do not have any risk factors of bleeding.

    COVID-19 affects all age groups; however, morbidity & mortality is several times higher in adults particularly in those above the age of 50 years. Children have either asymptomatic or mild infection. The general practice is to first evaluate any new vaccine in older population and then age reduction is done to assess the safety and effectiveness in paediatric population. The currently available vaccines have not been evaluated in children so far. There are some clinical trials now underway to test the effectiveness and safety of the COVID 19 vaccines in children.

    If Aadhar card is used as identification document, consent will be obtained and recorded.

    Through the portal, you can find out the list of available CVCs and dates and time of available vaccination slots, to book an appointment as per your choice. You will need an OTP verification prior to registration and a confirmation slip/token will be generated after registration. You will also get a confirmatory sms later.

    For all Private Hospitals, prior registration and appointment will be the only method of registration.

    For Government hospitals, a proportion of slots will be available for online registration and appointment, the rest will be kept for on site registration and vaccination.

    Appointments for any date for a Vaccination Center will be closed at 12:00 pm on the day prior to the date.

    Those who cannot get themselves registered online can contact their local Government health workers, who will help the beneficiaries to the Government CVC for on the spot registration, appointment, verification and vaccination on the same day. Please ask your nearest Governmen health care worker to guide you about the nearest Government CVC where COVID vaccination will be available and the days of the week when this will be available. You need to carry your mobile phone and a photo identification document to get yourself vaccinated.     In the initial phase, the COVID-19 vaccine will be provided to the priority group - Health Care and Front-line workers. The second phase of vaccinations, starting March 1, 2021 will allow for all Indians above the age of 60 and Indians between the age of 45 and 59 with comorbidities to be vaccinationated. Registrations will be allowed on the Co-WIN 2.0 Portal, Aarogya Setu app etc. The comorbidities that make a person eligible for vaccinations are listed below:

    • Diabetes (> 10 years OR with complications) AND Hypertension on treatment
    • Kidney/ Liver/ Hematopoietic stem cell transplant: Recipient/On wait-list
    • End-Stage Kidney Disease on hemodialysis/ CAPD

    There are a few bleeding disorders like 'haemophilia'. These persons should take the vaccine under the supervision of their treating physician. Patients who are admitted in hospital or ICU and have bleeding problems should delay the vaccination till they are discharged. However, several people with heart and brain disorders are on blood thinners like aspirin and antiplatelet drugs. They can continue with their medicines and have the vaccines.   • What is the safety and efficacy of the vaccines used in the country?

    To ensure that a vaccine is safe, we need to try it on a large number of people. The vaccine developers have not reduced the sample size at any stage of clinical trials rather it was bigger than what we usually test a vaccine on.

    When a vaccine is tested, most of the adverse events or unwanted effects, if any, occur in the first four to six weeks of its administration. So, in order to ensure that it is safe, we keep a close watch, for the first two-three months, on the people it has been given to. This data help us decide if a vaccine is safe. All concerned in the line of vaccine development, testing and evaluation have followed these procedures to the T. Both Indian vaccines are considered safe on this yardstick.

    As for the efficacy of the vaccine, we need time to tell how effective a vaccine is. All the global agencies have set the benchmark that only those vaccine candidates which show an efficacy of at least 50-60% will be considered. Most of the vaccines have shown an efficacy of 70-90% within the short period of two to three months of observation. Besides when a vaccine is given an emergency use authorizations/permission for restricted use, as in the case of the COVID-19 vaccine, the trial follow-up continues for one-two years to assess the total duration of protection the vaccine will provide.

    Ayush-64 is an Ayurvedic formulation, developed by the Central Council for Research in Ayurvedic Sciences (CCRAS), the apex body for research in Ayurveda under the Ministry of Ayush. Originally developed in 1980 for the management of Malaria, this drug has now been repurposed for Covid 19 as its ingredients showed notable antiviral, immune-modulator and antipyretic properties. The in-silico study done on Ayush 64 showed that 35 out of 36 of its Phyto-constituents have high binding affinity against COVID 19 virus. The formulation has also shown very promising results in Influenza like illnesses.   • What is Omicron and what makes it a variant of concern (VoC)?

    It is a new variant of SARS-CoV-2 that has recently been reported from South Africa on 24th November 2021 called as B.1.1.529 or Omicron (based on Greek alphabets like alpha, beta, delta etc). This variant has shown a very large number of mutations, especially more than 30 on the viral spike protein, which is the key target of the immune response.


    Given the collection of mutations in Omicron, which earlier individually have been associated with increased infectivity and/or immune evasion, and the sudden rise in number of positive cases in South Africa, World Health Organization has declared Omicron as a Variant of Concern (VoC).

  • Can the currently used diagnostics methods, detect Omicron?

    The most accepted and commonly used method of diagnostic for SARS-CoV2 Variant is RT-PCR method. This method detects specific genes in the virus, such as Spike (S), Enveloped (E) and Nucleocapsid (N) etc to confirm the presence of virus. However, in case of Omicron, as the S gene is heavily mutated, some of the primers may lead to results indicating absence of the S gene (called as S gene drop out). This particular S gene drop out along with the detection of other viral genes could be used a diagnostic feature of Omicron. However, for final confirmation of the omicron variant genomic sequencing is required.

Hence vaccines are expected to still offer protection against severe
